For a pet household
Pet Records and Emergency Pack
A record pack one household fills in by hand and keeps where a sitter, a family member or an emergency vet can find it: who to call first, your own veterinarian and the clinic that is open at night, who holds a key, who may decide what, and a signed authorization for treatment.
Then one record set for each pet — who they are, their health and medication, how they are fed, their routine and how they are handled — plus supplies and food storage, medication and incident logs, travel and boarding notes, what to do if a pet goes missing, with a poster to copy, and a two-week care chart.
What it covers
the parts in order
- household and contacts
- the record set you print again for each pet
- supplies, food storage and routines
- the logs
- the missing-pet section with the poster to copy
- away from home, with travel, boarding and handover notes
- the two-week care chart for whoever is in charge
- the authority pages — the emergency authorization and the card for the door
- and keeping it current, with an annual update and a change log
What you get
Each pack is a printable PDF you print at home, as often as you like. Choosing a pack takes you to Stripe's own secure checkout, which shows you the amount before you confirm anything, and your download is ready as soon as the payment goes through.
The forms inside are blank on purpose. Everything that depends on you — your vet, the clinic that is open at night, your phone number, your address, your rates, the wording of your own policies — is yours to fill in, so nothing in a pack invents a business or a price for you.
If a pack does not fit the way you care for your animals, tell us. We look forward to feedback to improve or add to our packs.
